Ssi Application

The Social Security Administration created the Supplemental Security Income (SSI) program to assist disabled and blind senior citizens over the age of 65 who are already receiving Social Security benefits but still suffers from low-income levels and limited resources. To begin the process of receiving SSI, a person must fill out an SSI application. These applications are available both online at the government's Social Security website or at Social Security offices across the nation. When applying general personal information is needed. This includes a Social Security card or record of a Social Security number, a birth certificate or proof of age, a mortgage or copy of an apartment lease, payroll slips and bank books and other records about things an applicant owns, medical contact information for those applying due to disability or blindness and proof of Citizenship or eligible non-citizen status. Parents, guardians or some third parties may fill out applications for blind or disabled children under age 18.
