Ss-5

An SS-5 is the name for the general application for a Social Security Card. This application can also be used to replace a lost Social Security Card or request a change of the information contained on the present Social Security Card. The application must be filed at the local social security office and the applicant must provide documentation of his birth, his age and his identity when making the request to be issued a Social Security Card.

Fast Facts

  • A U.S. hospital record of the applicant's birth, created at the time of the birth will be accepted as valid documentation.
  • To prove U.S. citizenship, an applicant must provide his U.S. birth certificate, his passport, a Consular Report of Birth, Certificate of Citizenship or Certificate of Naturalization.
