File Social Security Disability

When you are ready to file for Social Security disability benefits (SSD), you are required to bring proof of any medical treatment you have received from doctors, hospitals, therapists, and other medical professionals. While you may be eligible for SSD now you become disabled, benefits do not begin until six months after your disability began. If you earn more than $500 per month during the time you claim disability, you will be ineligible to receive benefits. To file your claim and determine eligibility, call the Social Security Administration (SSA) at 800-772-1213, or visit www.ssa.gov. Along with your application, you will need to show a certified copy of your birth certificate, your social security card, W-2s for several previous years' tax returns, medical reports, and lab reports, along with the contact information for any medical personnel or facilities involved in your care.
